PIC APPLICATION FORM
  • With a single application, applicants can apply through PIC to a variety of projects and programs listed in the PIC Directory. 
  • Applicants need to refer to the PIC Directory for information about attendant service projects you can apply in this application. 
  • Applicants need to complete all sections of the PIC Application Form and return to PIC. 

PIC DIRECTORY

  • PIC Directory provides essential information for PIC applicants.  It includes information about attendant service options you can apply in this application as well as location of projects and contact information of attendant service providers.
  • Applicants are advised to keep this directory as it is a valuable resource to refer to when completing and updating their PIC application.
