Are you a youth 16 to 29 with a physical disability finding the transition to adult services difficult? Want to find out more about how to live on your own? Need help finding a job? Or are you looking to get more involved in your community? With the help of the Ontario Trillium Foundation, Ontario March of Dimes and the Easter Seals Society are piloting a new service called the Life Stage Transitions project. They work to help you find resources that grow up with you.
